Seconds matter – training, equipment maintenance, and effective leadership are critical!

Several BSEE Safety Alerts have just been released. Of particular importance to those interested in deepwater drilling is the attached alert describing two separate Emergency Disconnect Sequence (EDS) incidents.

The EDS (see the diagram above) is a critically important safety protocol that ensures that a well is sealed and the riser and rig are disconnected from the blowout preventer in the event of a well control emergency, unforeseen weather/ocean conditions, loss of power, or positioning system malfunction. Note that the Macondo blowout could have been prevented if the Deepwater Horizon crew had activated the EDS in a timely manner.

The two EDS events cited in the Safety Alert were presumably the March 28, 2025 and March 5, 2024 incidents investigated by BSEE district offices. The drillships were the Stanley Lafosse and the Deepwater Poseidon The investigation reports provide detailed information on these incidents.

Unintended riser disconnects not associated with EDS activations are a related safety and pollution concern that necessitated the issuance of a 2000 Notices to Lessees that was subsequently updated:

According to BSEE, there is a recurring trend of equipment misuse contributing to fire and explosion hazards during offshore oil and gas operations in the Gulf of America.

Workers have used tools not rated for electrical work on live circuits (Figure 1) and mismatched hydraulic or pneumatic tools for high-pressure systems (Figure 2). In several cases, non-intrinsically safe hand tools were used in explosive atmospheres, including mudrooms and drilling floors.

As explained in the attached Safety Alert, BSEE’s risk-based inspection program has identified deficiencies in safety device bypass practices including:

  • inadequate documentation
  • inoperative data history software
  • bypassing more devices than is necessary
  • bypassing devices for longer than necessary
  • missing audit documentation
  • mistakenly bypassing the entire safety system during production

The regulations restricting the bypassing of safety devices are core elements of OCS regulatory and operator management programs. Because they are critical to process safety, these requirements are widely supported and strictly enforced.

Historically, falls are the most common cause of offshore injuries and fatalities, and hazardous grating is a leading contributing factor to these incidents.

BSEE’s risk-based inspection and safety alert programs have effectively drawn attention to grating risks. Attached is a recent alert describing a grating incident that could have been fatal.

A worker installing a pump in a skid above unsafe grating was kneeling on scaffolding boards. The tip of his boot was on the corroded grating when it suddenly gave way. The worker was able to grab a nearby section of piping to support himself. The 36″ x 36″ piece of grating collapsed and fell into the water.

On Sept. 16, 2024, a routine helicopter approach at an offshore facility nearly resulted in a serious accident due to a failure to follow proper helideck procedures. Before landing, the helicopter pilot visually confirmed that a nearby crane was securely stowed and stationary (Figure 1). However, as the helicopter neared the helideck, the crane operator unexpectedly raised the crane boom, bringing it alarmingly close to the landing area as the helicopter was 10 feet from touchdown. The pilots swiftly executed a go-around maneuver, successfully avoiding a collision and ensuring the safety of the crew and passengers onboard.”

Good work by BSEE in continuing to identify and address helideck safety issues. This is the 4th helideck safety alert issued in 2024.

Meanwhile, why are we still waiting for the final NTSB report on the tragic helideck crash that occurred 2 years ago?

The 3rd quarter update by Jason Mathews and a followup inquiry confirm that there were no work-related fatalities associated with US OCS oil and gas operations in 2023! This major achievement deserves public recognition given that the zero fatality goal has long eluded offshore operators, contractors, and regulators.

In a proper safety culture, continuous improvement is the primary goal, and both good and bad outcomes must be carefully assessed. The 2023 zero-deaths milestone is thus tempered by life threatening incidents such as those described in the attached safety alert and investigation report. Address these issues, identify other potential problem areas, and continue to drive the culture forward. Be proud and confident through training, planning, and achievement, but be wary!

Consistent with the findings of their inspections and data gathering (as discussed further here), BSEE has published a safety alert (attached) that identifies significant shortcomings in medical evacuation planning and performance.

The findings suggest that a renewed focus on medevac preparedness should be an immediate industry priority. Note the evacuation time, supply, training, and other planning issues summarized in the BSEE alert. Also note the helideck safety issues that were identified. These issues are particularly troubling in light of last December’s fatal crash.

The most common causes of offshore fatalities and serious injuries, falls and being struck by equipment, receive little media attention because there is no blowout, oil spill, or fire. However, these are often the most difficult types of incidents to understand and prevent. Human and organizational factors predominate, and prevention is dependent on a strong culture that emphasizes worker engagement, awareness, teamwork and mutual support, effective training and employee development, risk assessment at the job, facility, company, and industry levels, stop-work authority, innovation, and continuous improvement.

This new BSEE Safety Alert addresses such a fatal incident on the Pacific Khamsin drilling rig, and makes recommendations that have widespread applicability.

Incident summary:

While unlatching the lower Marine Riser Package from the Blowout Preventor in preparation for ship relocation, a crewmember was lifted into the air after being struck by a hydraulic torque wrench (HTW), hitting a riser clamp approximately six feet above the elevated work deck before falling to the rig floor. The crew member was given first aid and transported to the drillship’s hospital, where he was later pronounced deceased.
