I am still puzzled about the past DNV accident investigation report , the conclusion indicate that a possible cause was that the drill pipe was located alongside the wall of the BOP (not centered) and therefore the current BOP system did not permit the 2 rams to sweep the cross section and shear the pipe in a clean way ? the mangled pipe remained between the close ram blocks , leaving a gap for the well to continue flowing. The shape of the rams or force necessary was unsufficient to push the deflected or bent pipe in the center ?
A lot of disagreement on this point is to follow suit and will be forthcoming from various sources.
“IF” this conclusion is the correct one, then what BP is proposing re: item 1 increase the number of shear rams does not change anything to the basic problem of off center line pipe cutting , and could happen again.
Pipe could be damaged and bent, small pipe can be jammed up , and as suggested on Macondo deflection of the pipe was due to pressure and the pipe “anchored” from moving upwards by a partially close bag preventer above the shear rams cavity.
So far have not seen any informations or regulations proposing any alternative solutions to this type of BOP.
JL, The BOEMRE investigation report is to be released by 27 July. Hopefully, that document will provide additional insight on the BOP failure.
